If you are new to Impact ABA Therapy, we’re excited to start this journey with you! To help us better understand your child's needs and streamline the onboarding process, please complete the following forms:

• Client Waitlist Form - Join our waitlist and be the first to know when a spot becomes available. 
• Client Intake Form - Provide us with important details about your child so we can tailor our services to best support their progress.
• HIPAA Authorization Form – Authorize us to securely share and receive important information (like assessments, treatment updates, or insurance details) with other providers, schools, or agencies involved in your child’s care.
